Medical Report

medical report is a comprehensive report that covers a person’s clinical history. A medical report is a vital piece of evidence that can validate and support your claim for medical-related benefits.

Medical reports can be obtained from the Medical Record Department on the 3rd floor. It is required when applicants/patients apply for Insurance, EPF and SOCSO claims, as well as other matters.

 

HOW TO OBTAIN MEDICAL REPORT 

Insurance Related Reports

(Hospitalisation/Personal Accident/Outpatient claim/Attending Physician’s Statement/Medical Attendant Report/Questionnaires)

Procedure:

  1. Obtain the relevant form from your insurance company or insurance agent.  There are two parts to the form Part 1 and Part 2; complete Part 1 of the form if available.
  2. Submit an original copy of written consent of the patient or authorised next of kin is required for the release of medical information to person/s not otherwise authorised to receive this information.
  3. A nominal fee ranging from RM80.00 to RM500.00 is charged. An official receipt will be issued.
  4. The duration of processing the application is 28 working days (excluding Sundays and Public holidays) from the date of submission.
  5. The applicant will be notified upon completion of the report.

For the withdrawal of EPF Savings Report/SOCSO Benefits

Procedure:

  1. Obtain the relevant form from the EPF office/SOCSO office.
  2. Submitting an original copy of the written consent of the patient or authorised next of kin is required for the release of medical information to a person not otherwise authorised to receive this information.
  3. A nominal fee of between RM80.00 to RM150.00 is charged.  An official receipt will be issued on payment.
  4. The duration of processing the application is 28 working days (excluding Sundays and Public holidays) from the date of submission.
  5. The applicant will be notified upon completion of the report.
