Medical Report

medical report is a comprehensive report that covers a person’s clinical history. A medical report is a vital piece of evidence that can validate and support your claim for medical-related benefits.

Medical reports can be obtained from the Medical Record Department on the 3rd floor. It is required when applicants/patients apply for Insurance, EPF and SOCSO claims, as well as other matters.

HOW TO OBTAIN MEDICAL REPORT

MEDICAL REPORT APPLICATION  

Insurance Related Reports

(Hospitalisation/Personal Accident/Outpatient claim/Attending Physician’s Statement/Medical Attendant Report/Questionnaires)

Procedure:

  1. Obtain the relevant form from your insurance company or insurance agent.  There are two parts to the form Part 1 and Part 2; complete Part 1 of the form if available.
  2. Submit an original copy of the patient’s written consent or authorised next of kin is required to release medical information to a person not otherwise authorised to receive this information.
  3. A nominal fee ranging from RM80.00 to RM500.00 is charged. An official receipt will be issued upon confirmation of the claim.
  4. The duration of processing the application is 28 working days (excluding Sundays and Public holidays) from the date of submission.
  5. The applicant will be notified upon completion of the report via phone call/emails

EPF / Sosco Reports Claim

Procedure:

  1. Applicants are to obtain the relevant form from the EPF office/SOCSO office.
  2. Submitting an original copy of the patient’s written consent or authorised next of kin is required to release medical information to a person not otherwise authorised to receive this information.
  3. A nominal fee of RM80.00 and above.  An official receipt will be issued upon confirmation of the claim.
  4. The duration of processing the application is 28 working days (excluding Sundays and Public holidays) from the date of submission.
  5. The applicant will be notified upon completion of the report via phone calls/emails
